Spongiform pustules from impetigo herpetiformis in children were examined by electron microscopy. Besides the ultrastructural features described by Rupec, close interactions between infiltrating neutrophilic leukocytes and keratinocytes was observed. Within the lesion, the cell membranes at the contacting surfaces of a leukocyte and a keratinocyte occasionally were partially invisible and, through this cytoplasmic continuity, the granules of the leukocyte seemed to move into the adjacent keratinocyte. Also a leukocyte was seen to invaginate deeply into a keratinocyte. This ultrastructure is likely to be further responsible for the light microscopic appearance of unicellular pustule formation.
