Despite the purported importance of the role of physical activity in the development of obesity in children, epidemiologic data addressing this issue are surprisingly limited. The study designs have primarily been ecologic and cross-sectional with few studies prospectively evaluating the relationship of physical activity to subsequent weight gain and obesity in children. As will be presented, there are two fundamental difficulties of the literature. The first is the difficulty of assessing physical activity and obesity in children. If there are problems in the reliability and validity of the measurement, then there is the concern that discrepancies seen in the literature may be because of measurement errors. The second difficulty is that of study design in that the vast majority of the studies have been either ecologic or cross-sectional. With these designs, it is impossible to determine what comes first, obesity or maturity. The current paper, therefore reviews what is known about the epidemiology of physical activity and obesity in children. We argue that the results have been inconsistent not only because of measurement errors or weak study designs, but also because total energy expenditure, not just that due from physical activity, should be assessed in order to find the true physical activity-obesity relationship.
