Dear Sir, Diabetic nephropathy, which is characterized by persistent proteinuria (i.e., total urinary protein excretion > 0.5 g/24 h), contributes to the leading causes of morbidity and mortality in diabetic patients [ 11. Several lines of recent evidence suggest a beneficial effect of strict metabolic control on development of incipient nephropathy in IDDM patients [2]. An improvement of glycaemic control has also been shown to decrease urinary albumin excretion in newly diagnosed NIDDM patients [3]. Nevertheless, no such beneficial effect of glycaemic control has been demonstrable on the progression of kidney impairment in IDDM patients with overt nephropathy [4,5]. There seems little information available as to the clinical significance of glycaemic control in the progression of the proteinuric stage of nephropathy in NIDDM patients, despite the fact that the majority of diabetic patients are NIDDM.
