Evaluation of a Training Program in Professional Autonomy Intended to Improve the Midwifery Practice Competence of Mid-career Generalist Midwives

The aim of the present study was to investigate the appropriateness of training content by evaluating a training program in professional autonomy intended to improve the practical competencies of mid-career generalist midwives. Subjects were 43 mid-career generalist midwives who took seven training courses in professional autonomy. They self-evaluated their abilities in the content covered by the courses immediately after, 3 months after, and 1 year after taking the courses. Of the seven training courses in professional autonomy, their self-evaluations rose in the following four courses: “Leadership from a management perspective,” “Management of the quality of midwifery care,” “Use of the Guidelines for Obstetrical Practice in Japan,” and “The process of ethical decision-making.” However, no subjects reached a self-evaluation level of 4 or 5 in these courses, corresponding to level 3 on the Clinical Ladder of Competencies for Midwifery Practice. During the training program, it was found that utilizing practice cases was effective, and lecture was necessary to compare proper practices with the midwives’ current practices. In order to improve the abilities covered by the training courses, it is necessary for midwives to practice them in the clinical setting.
