Fluctuation of non-cycloplegic autorefraction readings observed in patients with hyperopic anisometropic amblyopia

<p>[Purpose] Although non-cycloplegic autorefraction readings varies by the effect of voluntary accommodation, very large fluctuation of the values is sometimes found only in one eye. We tried to find the factor about the large fluctuation in hyperopic children.</p><p>[Materials and Methods] Subjects were untreated hyperopic children, aged 3 to 8 years (mean age, 6.0±1.7years) who visited Kawasaki Hospital, Kawasaki Medical School between January 2013 and September 2015. In 28 eyes of 14 children with anisometropic amblyopia, and 20 eyes of 10 children without amblyopia, non-cycloplegic refractive readings were obtained with an autorefractometer, RC-5000® (TOMEY). The fluctuation was compared between the groups. We also analyazed the relationship between the amount of the fluctuation and cycloplegic refractive readings, the amount of refractive change induced by cycloplegia, anisometropia, age, and visual acuity. The fluctuation was defined as a standard deviation of 5 successive readings.</p><p>[Results] The fluctuation of the spherical readings in amblyopic eyes and the fellow eyes was 0.31D and 0.14D, respectively. That of dominant eyes and non-dominant eyes in non-amblyopic children was 0.11D and 0.11D, respectively. Amblyopic eyes showed a larger fluctuation (p<0.01). The fluctuation of the cylindrical readings was 0.11D, 0.12D, 0.00D and 0.11D, respectively. Fluctuation of sphere found in amblyopic eyes was significantly correlated with the amount of refractive change induced by cycloplegia.</p><p>[Conclusion] Large fluctuation of refractive readings of the monocular eyes in non-cycloplegic autorefraction probably indicates the existance of amblyopia.</p>
