The Ievels of plasma benzodiazepine (BZ) -1ike substance in patients with hepatic encephalopathy (HE) are recently reported to be increased in serum and cerebrospinal fluid and this is assumed as one of the cause of HE.<BR>We investigated the efficacy of flumazenil, a specific antagonist of brain BZ receptor, in ten patients of alcoholic liver cirrhosis with HE. Flumazenil was administered intravenously as a bolus. Changes of HE grade and electroencephalogram (EEG) were recorded. Improvement of HE was observed in six patients, two patients had no relapse of HE, but in the other four patients, the effect continued for 30-120 minutes and then HE recurred. Improvement of EEG was noticed in six patients. Flumazenil is not a therapy for liver failure itself. However, it is useful to control the patients with HE.
