Evaluation of x-ray findings for assessing the risk of stomach cancer using serum <I>Helicobacter pylori</I> antibody and the pepsinogen method

In the current study, we examined the association between the appearance of the gastric area on the posterior wall of the middle of the stomach and the width of the gastric folds, as revealed by x-ray images of the stomach, and the serum Helicobacter pylori (Hp) antibodies and the serum pepsinogen (PG) method in the same individuals. The subjects were 140 individuals who underwent direct radiography of the stomach during a medical checkup at this Center, in whom serum Hp antibodies and PG were measured in optional tests on the same day. The gastric area was classified into the classes B-0 to B-3 based on its morphological appearance, and the classes B-1 and B-3 were further divided (B-1a and b, B-3a and b) based on the size of the gastric area. The results were as follows. Combining the gastric area classification and the fold width classification helped to identify individuals with both a gastric area of class B-1 or higher and a fold width of 3 mm or greater, and these individuals were deemed to be positive for Hp infection with a sensitivity of 75.0% and a specificity of 97.2%. When individuals who met either of the criteria were deemed to be positive for infection, this resulted in a sensitivity of 96.9% and a specificity of 85.9%. In addition, individuals with both a gastric area of class B-2 and a fold width of 4 mm or smaller were combined with individuals with a gastric area of class B-3 or higher and considered to be individuals with PG method(+). Individuals with other classes of gastric area or fold width were considered to be individuals with PG method(-). This resulted in a rate of concordance with the PG method of 91.9%. This study may indicate that Hp infection can be diagnosed and the risk of stomach cancer objectively assessed based on x-ray images of the stomach.
