- Establishment of a Core Outcome Set for Burn Care Research: Development and International Consensus

<p> 【目的】国際的に使用可能な熱傷治療研究のためのコア・アウトカム・セット (Core Outcome Set: COS) の選定.<br> 【デザイン】国際的コンセンサスの合意 (作成期間: 2017年4月~2019年11月).<br> 【方法】システマティック・レビューおよび関連文献から, 検討候補となるアウトカム (臨床評価項目) を抽出した. デルファイ調査方法による共同意思決定作業を通じて, 各国の臨床家と研究者およびイギリス圏の患者と介護者が, アウトカムの優先順位決定作業に参加した. 匿名化されたフィードバック作業を通じて, コンセンサスの達成が目指された. まずアウトカム選定のための定義基準が事前に合意され, それに基づいた投票を行うためのコンセンサス会議が開催され, 最終的にCOSが決定された.<br> 【結果】データソースの調査から1,021のアウトカムが抽出され, ついでその中から88のアウトカム候補が特定された. 1回目のデルファイ調査には, 77ヵ国の医療従事者668名 (18%が低・中低所得国) とイギリス圏の患者または介護者126名が参加した. 1回目終了後, 1つのアウトカムが破棄され, 13の新しいアウトカムが追加された. 2回目の調査では69項目が破棄され, 31項目のアウトカムが最終のコンセンサス会議へと残された. 最終会議では, 事前に設定した選択基準 (閾値) に基づいて2回の検討と投票が行われ, 最終的に7つのコア・アウトカムが合意された (死亡, 特定の合併症, 日常生活動作能力, 創傷治癒, 神経障害性疼痛および掻痒感, 心理的ウェルビーイング, 学校または職場への復帰).<br> 【結論】このCOSには国際的に通用する普遍性があり, 熱傷領域で行われる介入試験において, プロトコル作成の前提になるものである. 今後の介入試験研究には, このCOSに準じた測定値または評価を含めることを推奨する.</p>
