Eye position was examined with the prism cover test in 76 patients (58 males and 18 females) with homonymous hemianopsia caused by intracranial disease but without low visual acuity in either eye from June 1997 through October 1998. Left homonymous hemianopsia was correlated with exotropia at the near (χ2 value, 4.039; p<0.05) and far (χ2 value, 4.586; p<0.05) eye positions. Initial examinations were performed less than 1 year after onset in 18 patients (15 males and 3 females). In these patients, eye position changed with time at the near but not at the far eye position. The correlation between the laterality of homonymous hemianopsia and that of the fixed, exotropic eye was not symmetrical. Patients with left homonymous hemianopsia had fixation of the right, left, or both eyes, but none of the patients with right homonymous hemianopsia had fixation of the left eye. The differences in the frequency of exotropia and the selective relationship between the laterality of the fixed eye and the laterality of homonymous hemianopsia suggest there is lesion localization of the mechanism for maintaining eye position in the human brain.
