Description

Background. Accessory cardiac bronchus (ACB) is a rare anomaly of the tracheobronchial tree which is usually discovered incidentally during examinations for other thoracic diseases. Occasionally, however, patients become symptomatic, showing recurrent episodes of infection or hemosputum. This report describes 2 cases of symptomatic ACB. Case 1. A 62-year-old man sought treatment because of hemosputum. Bronchoscopy revealed ACB. The orifice contained blood and the mucosa showed redness and swelling, and therefore an inflammation of the ACB was considered to be the cause of the hemosputum. Conservative therapy was effective. Case 2. A 61-year-old man presented with precordial pain and high fever. Chest CT and bronchoscopy showed a mediastinal abscess extending from the ACB. Antibiotic therapy was effective and he has shown no sign of recurrence. Conclusion. ACB is rare, but occasionally it can cause clinical symptoms. Therefore, it is important for clinicians to be aware of ACB.
