A Case of Bilateral Congenital Diaphragmatic Hernias

Description

A 6-month-old male infant was admitted to our department because of poor weight gain which had been found since the age of 6 weeks. Physical examination showed tachypnea, cyanosis, and malnutrition. Chest X-ray revealed elevation of bilateral diaphragms, and fluoroscope revealed normal diaphragmatic movement. Under the suspicion of bilateral congenital diaphragmatic hernias or eventration, laparotomy was performed. At laparotomy, posterolateral defect in the both diaphragms and herniation of the stomach, small bowel, colon, spleen, pancreas, and the right lobe of the liver were revealed. When the viscera was returned to the abdominal cavity, a hernial sac was appeared in both sides. The operative diagnosis was bilateral posterolateral (Bochdalek) hernias with hernial sac. The sacs were excised and the posterolateral defects were repaired by direct suture. Pathological examination revealed that the bilateral hernial sacs were consisted of collagen fiber bundles. The postoperative course was uneventful. One year has passed since the operation, and he is doing well now.
