Description
A 62-year-old man visited our emergency clinic because of epigastralgia and right lower quadrant pain on Saturday night, January 27 2007. The patient had undergone an operation for appendicitis 30 years before entry, when only drainage had been conducted. The hematologic and blood chemical values showed inflammatory changes. US failed to make diagnosis. The duty doctor at the night diagnosed the case as acute enterocolitis based on abdominal CT findings and started to administer antibiotics. On the next day he had tenderness in the right upper quadrant of abdomen. Two days later when roentgenologists interpreted the CT findings, deviation of the cecum upward the ileocecum and the swollen appendix containing appendicolith were revealed. Therefore the patient was diagnosed as to have acute appendicitis with mobile cecum. He was operated on via a pararectal upper abdominal incision to deal with the base of the appendix. Laparotomy showed that the cecum was firmly adherent to the ascending colon. After adhesiolysis, the appendix was resected with a part of the cecum.<BR>Mobile cecum is not uncommon and then we should keep the disease in mind in the diagnosis of acute abdomen. We describe the patient who underwent surgery via an appropriate skin incision after the diagnosis of acute appendicitis with mobile cecum was made by CT.
