A Case of Abscess of the Fallopian Tube with Peritinitis Caused by a Fish Bone Penetrating the Sigmoid Colon

Abstract

An 85-year-old woman complaining of a one-month history of lower abdominal pain was brought into our hospital with a diagnosis of perforative peritonitis. When she was first seen, the body temperature was 38.5°C and there was tenderness in the entire abdomen. Hematological examination showed significant inflammatory reaction, and abdominal CT scans showed free air in the abdominal cavity with multiple sigmoid diverticula with wall thickening and intra-abdominal abscess. Therefore, emergency laparotomy was performed because of perforative peritonitis by sigmoid diverticulitis on the same day. Laparotomy demonstrated heavy purulent ascites fluid and a foreign body which penetrated the sigmoid colon and the right fallopian tube. Right salpingo-oophorectomy and partial resection of the sigmoid colon with temporary colostomy were performed. The foreign body was a fish bone, 3.5 cm in diameter. Subsequently, the diagnosis of penetration caused by fish bone was made.<BR>To our knowledge, fish bone penetration of the fallopian tube with abscess formation has not been reported in the literature.
