The therapeutic ranges of anticoagulants and the clinical effect of antiplatelets were determined in mechanical valve replacement patients. The therapeutic ranges of anticoagulants in 100 patients (aortic valve replacement 50, mitral valve replacement 50) were 22.8±6.6 % with thrombotest (TT) and 1.83±3.1 with TT-INR. These values are considerably lower than the international standard of 2.5 to 3. 5 for prothrombin time (PT)-INR. Platelet aggregation was compared between 50 patients receiving 300 mg/day of ticlopidine and 50 patients receiving 81mg/day of aspirin. ADP aggregation was significantly depressed in the formergroup, but there was no difference in collagen aggregation between the two groups. Neither was there any significant difference in the frequency of the thromboembolic complication nor the bleeding tendency which was observed during the follow-up period.
