Description
A 46 year-old woman with myelodysplastic syndrome had been treated from 1983. In December 1993, she presented a painful nodule on her left forearm. This nodule subsequently ulcerated and then developed into an asymmetrical ulcer with black crust surrounded by erythema. Histologically, the epidermis was found to be necrotic and a dense infiltrate of neutrophils was present in the upper and lower dermis. From these findings, the nodule was diagnosed as pyoderma gangrenosum. The lesion disappeared with prednizolone therapy. A new nodule, however, developed on the head accompanied by fever in July 1994, and despite a repeat of the steroid therapy, there was no improvement. The patient died soon after from a cerebral infarction.<BR>In addition to a case report, we discussed the definition and scope of “neutrophilic dermatoses”.
