- Cytokeratin (CK 7, CK 20) and MUC 5 AC expression in ampullary carcinoma

Ampullary carcinoma can be divided into the pancreatobiliary differentiation and intestinal phenotypes. The former is usually invasive and has a lower 5-year survival rate. In this study, ampullary carcinoma was immunohistochemically investigated by cytokeratins (CKs) and MUC 5 AC, which were found in the non-tumorous duodenal papilla and so-called overreplacement, or distended glands of the duodenum, respectively. Ampullary carcinoma specimens were obtained from 16 patients who underwent pancreatoduodenectomy. Immunoreactivities for both CK 7 and CK 20 as well as MUC 5 AC were found in 11 and 9 of the 16 cases, respectively. Among these positive specimens, CK 7 and CK 20 demonstrated reverse immunoreactivity in 4 cases each,8 cases in total, with both being immunopositive in 7 cases and both immunonegative in one case that was positive for MUC 5 AC. Immunoreactivity for MUC 5 AC and CK 7 was coincident in 12 cases, while the coincidence for MUC 5 AC and CK 20 was obtained only in 4 cases. Therefore, ampullary carcinoma showed various phenotypes, as follows: pancreatobiliary differentiation in 4 and intestinal phenotype in 4, dual immunoreactivity as described above in 7 and dual negativity in one.
