- 第13回臨床不整脈研究会 Dead end pathwayの存在を示唆する電位がHis束電位記録領域で記録された房室(HV)ブロックの1例
Description
症例は79歳男性. 2:1房室ブロック精査のため心臓電気生理検査を施行した.電極長2mm10極カテーテルを用いHis束電位記録領域をマッピングした.ブロック部位はHV間であった.しかしながら,His束電位記録領域遠位側で記録されたsharpなfrist potentialから25msec遅れてHi束電位記録領域近位側においてdullなsecond potentialが検出された.心房(A)プログラム刺激でAfifirst potential間,A-second potentia1間ともに減衰伝導を示し,ATP静脈内投与により両者とも伝導時間は延長した.しかしながら,procainamide静脈内投与ではHis東電位記録領域の前述した2つの電位に明らかな変化を認めなかった. His束電位記録領域近位側においてHis束電位より遅れて記録されたATP感受性電位は, deadendpathway由来の電位と考えられた.
