Nonocclusive mesenteric ischemia (NOMI) is a rare disease characterized by high mortality. A 58-year-old woman complained of abdominal pain and nausea 7 days after aortic root replacement and total arch replacement for aortic dissection.<BR>Emergency computed tomography revealed portal venous gas, free air in the abdominal cavity, and pneumatosis intestinalis. No apparent occlusion of the mesenteric artery was observed. We suspected NOMI and performed an emergency laparotomy. Intraoperative findings revealed discontinuous necrosis from the jejunum to the terminal ileum. Resections of the ischemic small intestine and ileocecum, and double-barreled jejunocolostomy were performed. On the 43rd day postjejunocolostomy, we performed jejunocolostomy closure. She was discharged from our hospital on the 99th day post jejunocolostomy. Here, we report a survival case of NOMI after aortic dissection repair.
