The Contribution of Endurance Fitness, Obesity, Insulin Resistance, Sex Hormones, and Sex Hormone-Binding Globulin (SHBG) on Lipids and Lipoproteins in Japanese Men with IGT and NIDDM

Abstract

The purposes of present study were to evaluate the profiles of sex hormones level and lipid metabolism and to investigate the relationships of lipids and lipoprotein metabolism to sex hormones, sex hormone-binding globulin (SHBG), insulin resistance evaluated by HOMA model, endurance fitness (estimated maximal oxygen uptake; VO2max) and some obesity indices in Japanese men with age-matched impaired glucose tolerance (IGT; n=19, 49±11 yr), non-insulin-dependent diabetes mellitus (NIDDM; n=40, 48±7 yr) and non-diabetic healthy control (n=167, 46±9 yr) groups. Criteria in Japan Diabetes Association was used in classification for diabetes mellitus. Body mass index (BMD and waist-to-hip ratio (WHR) in the both patient groups were significantly incresaed as compared with than that of control group (p<0.05). The levels of the serum free testosterone and high density lipoprotein cholesterol (HDL-c) in IGT and NIDDM groups were significantly lower than that of control group (p<0.05). Insulin resistance score increased in the order NIDDM>IGT>control. Siginificant correlations were observed between lipid metabolism and some obesity indices, VO2max, insulin reistance and SHBG in all subjects and patient groups. In multiple linear regression analysis in all subjects, VO2max and/or insulin resistance are significant independent varaibles for variance in lipid metabolism. In only patient group, to2max and/or SHBG are siginificant independent varaibles for variance of lipid metabolism. From these results, it was suggested that lipid abnomality may exist in patients with early phase of NIDDM and/or IGT, and decreased insulin sensitivity and lower endurance fitness level were contributed to their lipid abnomality.
