Abstract

A 31-year-old man was transported to our hospital following a traffic injury. The patient had bilateral femoral diaphyseal fractures and open pelvic fractures, as well as an intra-abdominal injury. Steel wire traction was performed as a standby surgical strategy for the femur fracture, but on the second day of admission, tachycardia, hypotension, impaired oxygenation, impaired consciousness, and fever were observed, and a diagnosis of fat embolism syndrome was made. An urgent intramedullary nailing procedure was performed as stabilization of the fracture was considered necessary. The patient’s loss of consciousness did not improve, and he was transferred to another hospital on the 31st day. The early development of fat embolism syndrome is associated with a higher severity of disease ; therefore, fat embolism syndrome should always be kept in mind when treating patients with polytrauma.
