Description

A 68-year-old woman presented to the emergency department complaining of epigastric pain of acute onset. Contrast-enhanced CT of the abdomen revealed closed-loop obstruction of a hypo-enhancing small bowel, suggestive of strangulated bowel obstruction caused by the appendix. We performed emergency laparoscopic surgery. Laparoscopic exploration revealed that the tip of the appendix was adherent to the abdominal wall forming a band, and the ileum protruded through this defect forming an internal hernia. When the adhesion between the appendix and abdominal wall was dissected, the strangulation was released and the blood flow improved; no intestinal resection was needed. Therefore, we performed laparoscopic appendectomy alone. Her postoperative course was uneventful, and the patient was discharged 9 days after surgery. We present a case of strangulated bowel obstruction due to adhesion of the appendix treated by emergency laparoscopic surgery.
