Social and Psychiatrical Background of the Abusers of a Nonprescription Cough Cure Solution (Alias 'Bron')

The abuse of nonprescription cough syrups has been noted in the 1980's in Japan. Although several kinds of cough cure solutions are available, the brand most abused is S. S. Bron (referred to in this paper as Bron). The main ingredients of Bron are methylephedrine, codeine and caffeine. We made case studies on 33 Bron abusers found in six specialized institutions in Kanto and vicinity from 1983 to 1988, and compared the epidemiological and psychomedical findings with those of 47 abusers of thinner and 44 abusers of Methamphetamine. Generally, the patients abusing Bron were from more affluent families and they had more foreign friends than those in the other two categories. A reason given as a motivation for use of the cough syrup was "just for fun". In 12 of the patients of Bron abuse (36.4%) hallucinatory-paranoid state was the only psychotic symptom noted throughout the period of abuse, whereas only an affective disorder was observed for the duration of abuse in 13 patients (39.4%). A higher ratio of patients with affective disorder was shown in abusers of Bron than in those of thinner or Methamphetamine. Clear autonomic nervous symptoms appeared in 16 patients of Bron abuse, including 15 with affective disorders during the period of abuse. However, no clear autonomic nervous symptoms were indicated in patients with hallucinatoryparanoid state.
