Penoscrotal Approach for Implantation of Penile Prosthesis for Impotence

A technique of penoscrotal approach to implant paired penile prosthesis was developed and evaluated in 20 impotent patients during the past 2 years and 5 months period. With this approach one can avoid the technical inconveniences of deep perineal fat incision and incision of the ischicavernous muscles, which routinely encounter at the conventional perineal approach procedure. Also, one can avoid the damage to the neurovascular bundle of the penis at the dorsal approach procedure. Of 23 patients, including 3 patients who were operated previously by conventional perineal approach, 20 patients have been under our consecutive follow up observation and all of them showed successful results up to present. The semi-rigid and hinged Finney type paired prosthesis were used in all the cases. The average length of the prostheses implanted was 192. 3 mm, varied from 163 to 218mm. Mild discomfort in perineum has persisted in a patient who was operated by perineal approach, but none of the patients complained of the similar symptom in the penoscrotal approach group. Further use of the latter approach is anticipated and encouraged.
